Brain tumors are known as abnormal cellular growths in the brain, which can develop into a mass or lesion. They can be either benign, i.e., non-cancerous, or malignant, i.e., cancerous, and arise from different types of cells present in the brain. Brain tumors are classified according to their location, the type of cells involved, whether they are primary or secondary. The concept of brain tumors is well known; the organ associated with this term is the brain, the part of the body that can cause the condition- disruption of the normal functioning of the brain.
The brain is one of the most complex and essential organs of all human organs. Most human life functions are controlled and kept under control by the brain, including thinking and memory, the skill of moving different parts of the body, and regulation of how the body works. Abnormal cell growth in any part of the brain becomes an interference with many vital functions of the organ and leads to neurological symptoms. The symptoms vary greatly as they depend on both the size and position of the tumor, as well as whether it is malignant or benign. They include general headache, vomiting, nausea, personality changes, cognitive impairment, difficulty with coordination or balance, seizures, and abnormalities in the senses.
Brain tumors can arise in any type of cell in the brain. One of the most common classes of brain tumors can fall into the category of glioma and is derived from glial cells, which serve as scaffolding and nourishment for neurons. There are additional subclassifications to this, which are further divided into types based on the specific glial cell from which the tumor was derived: astrocytomas—for astrocytes—, oligodendrogliomas—for oligodendrocytes—, and ependymomas—for ependymal cells. Another common form of brain tumor is meningioma, which arises from the meningeal layers that cover and surround the brain and spinal cord. Mostly benign, they can cause tremendous health havoc if they get close and start putting pressure on adjacent structures of the brain.
Other possible tissues that can give rise to tumors include blood vessels, the pituitary gland, and the dura mater (the protective covering of the brain). Some tumors known as metastatic brain tumors are not primary brain tumors; they arise from cancer spreading to the brain from other parts such as the lungs, breast or skin. Metastatic tumors are usually more aggressive than primary brain tumors and are more difficult to treat.
Most brain tumors have no defined cause. However, several factors may predispose someone to develop a brain tumor. These factors include a family history of brain tumors, certain genetic syndromes, exposure to ionizing radiation, and environmental exposure to carcinogenic substances. Otherwise, most brain tumors develop for no known reason, and also, most individuals suffering from brain tumors have no risk factors.
The diagnosis of a brain tumor typically begins with a clinical evaluation by a physician who takes a close history and conducts a neurological examination and subsequently assesses the patient's symptoms. If a brain tumor is suspected, further findings are usually drawn from imaging studies. The most common imaging techniques for detecting brain tumors are magnetic resonance imaging (MRI), which creates very detailed images of the brain's anatomy, and computed tomography (CT) scans. A biopsy may be performed in some cases to sample a bit of tumor tissue for microscopic examination and a more accurate diagnosis.
The method of treating a brain tumor depends on factors such as the size of the tumor, the type of tumor, where it is located, and whether it is benign or malignant. In some cases, surgery is required to remove the tumor, especially in the case of severe pressure on vital parts of the brain or if it is in a location where it can be safely reached. However, brain surgery is highly complex due to the delicate nature of the tissues there, so not all tumors can be removed surgically. For example, some tumors occur in areas that control important brain functions, and some may be buried too deeply to safely gain access.
But for tumors that cannot be surgically removed, other treatments can be used. Radiation therapy is often the treatment of choice in brain tumors because it uses high-energy beams to kill or shrink tumor cells. Externally, radiation therapy can be done by delivering beams of radiation directly into the body of the tumor; or internally, where radioactive material is placed in or around the tumor. Chemotherapy is another available treatment option that attacks cancer cells or inhibits their growth using drugs that have severe side effects. In such cases chemotherapy can be given either orally, intravenously, or directly into the cerebrospinal fluid. Certain types of tumors may also be considered to undergo targeted therapy or immunotherapy, which aims to attack specific tumor cells or activate the immune system to fight cancer, respectively.
Apart from all those standard treatments, clinical trials and experimental treatments may provide some new avenues for treatment. A patient with a brain tumor may participate in a clinical trial to test a new drug, a new radiation protocol, or another treatment regimen. However, the availability of clinical trials varies among patients for a variety of reasons, mostly associated with the patient's condition and treatment characteristics.
The prognosis for individuals suffering from brain tumors is inconsistent. It depends on the type of tumor: some are malignant rather than benign, while others have been found at a particular stage or location. Generally speaking, however, benign brain tumors have a better prognosis than malignant tumors. Benign tumors can also be a problem, as sometimes they are quite small in themselves and may therefore put pressure on certain vital structures. Glioblastomas are malignant brain tumors, possibly the most aggressive and difficult to manage with regard to prognosis. Unfortunately, even after good advances in treatment, the survival rate for brain cancer is lower than for many other solid malignant diseases. Some patients, especially those with benign brain tumors or those diagnosed at an early stage, survive for a very large number of years after treatment.
The emotional and psychological aspects of having a brain tumor can range from fear, anxiety and uncertainty, among others. Patients and their families often find themselves struggling with such emotions. A diagnosis of a brain tumor usually requires adapting to a "new reality" that may involve life changes, including changes to daily activities and even relationships. Dealing with the physical characteristics of the tumor as well as the side effects associated with treatment may also prove difficult and requires the support of professionals, family and friends and those who offer counseling in such circumstances to help navigate the complex emotional landscape that comes with a diagnosis of a brain tumor.
Rehabilitation is very essential in the first month after surgery for all brain tumor patients or patients with neurological deficits as no surgical operation is performed for such patients. It includes techniques that include speech therapy and methods of physical and occupational therapy, where a patient regains lost ability as well as life. Long-term deficits include forms such as cognitive, emotional or physical, suffered from the long effects of brain tumor or treatment, which is more dependent on active input and continuous support by health care providers.
Currently, studies on brain tumors are underway and new treatments and diagnostic modalities are on trial. Developments in molecular biology, genomics and personalized medicine are expected to aid research on the genetic and molecular basis of brain tumors towards innovative personalized treatments. Most of the research trials conducted on immunotherapy are for purposes such as stimulating immune defenses generated against cancer. Although extremely difficult to treat, brain tumor research is producing results that improve the conditions of this unfortunate population and greatly improve their survival.
